RESUMO

Alzheimer's disease (AD) is a neurodegenerative disorder characterized by mechanisms including excessive deposition of β amyloid (Aβ), neuroinflammatory responses, and hyperphosphorylation of microtubule-associated protein (Tau protein). Currently, there are no effective clinical treatments available for AD. Traditional Chinese herbal drugs have gained attention for their potential to exert anti-AD effects through multi-component, multi-target approaches. As a traditional Chinese herbal drug with over 600 years of clinical use, Geranii Herba has substantial medicinal potential and wide application prospects. Geranium medicinal plants contain chemical components such as tannins, flavonoids, organic acids, and volatile oils. Research has indicated that various tannin compounds found in Geranii Herba possess pharmacological activities like enhancing learning and memory abilities, and improving cognitive function. These effects are linked to mechanisms involving anti-Aβ effects, Tau protein regulation, antioxidation, anti-inflammation, and inhibition of acetylcholinesterase activity. These compounds can act through multiple pathways and targets to inhibit neurodegenerative changes in neurons, thus effectively preventing and treating neurodegenerative diseases like AD. Based on relevant literature, this study focused on reviewing various tannin components in Geranium plants and their role in preventing and treating AD and identified potential drug components for treating neurodegenerative diseases such as AD by exploring the tannin components and their mechanisms in Geranium plants, thereby providing a theoretical foundation and research direction for further development and clinical application.

Objective:To measure the predictive value of miR-424 on the risk of sepsis complicated with acute respiratory distress syndrome(ARDS), and to explore its correlation with the prognosis of ARDS children.Methods:A prospective study was conducted.The data of children with sepsis (some complicated with ARDS), who were treated in Henan Provincial People′s Hospital (People′s Hospital, Zhengzhou University) were collected from February 2020 to February 2021.The incidence of ARDS and the fatality rate of ARDS children were recorded.Children were divided into survival group and death group according to whether death or not.The expression of miR-424 in peripheral blood mononuclear cells was detected by quantitative reverse transcription polymerase chain reaction (RT-qPCR). Pearson correlation analysis was made to investigate the relationship between the expression level of miR-424 and Pediatric Critical Illness Score(PCIS), Sequential Organ Failure Assessment(SOFA) score, oxygenation index(P/F ratio), C-reactive protein(CRP), procalcitonin (PCT), interleukin 6 (IL-6) and interleukin 8 (IL-8). The factors affecting the prognosis of ARDS children were analyzed by multivariate Logistic regression.Receiver operating characteristic curve(ROC)was used to assess the accuracy of miR-424 in early diagnosis of sepsis complicated with ARDS, and to measure the predicative value of miR-424 on the risk of death in sepsis patients with ARDS. Results:A total of 121 sepsis patients were included in this study, and 36 cases of them were complicated with ARDS.The expression level of miR-424(0.56±0.17)in the blood of sepsis patients complicated with ARDS was significantly lower than that of sepsis patients without ARDS (0.98±0.26)( t=8.776, P<0.001). The expression of miR-424 was negatively co-rrelated with IL-6( r=-0.627, P<0.01), IL-8 ( r=-0.651, P<0.01) and CRP( r=-0.472, P<0.05)in sepsis patients with ARDS.The expression of miR-424 was positively correlated with PCIS score( r=0.330, P<0.05), P/F ratio ( r=0.592, P<0.001) and albumin ( r=0.496, P<0.05) in sepsis patients with ARDS.The ROC showed that miR-424 [area under curve(AUC)=0.908, 95% CI: 0.856-0.960] could differentiate sepsis patients with ARDS from those without ARDS.Compared with that of the survival group(0.63±0.15), miR-424 of the death group decreased significantly(0.42±0.14)( t=3.890, P<0.05). The decreased miR-424 (AUC=0.845, 95% CI: 0.696-0.995)indicated an increased risk of death in children with ARDS.Multivariate Logistic regression analysis showed that miR-424( OR=0.001, P=0.033)and albumin ( OR=0.553, P=0.040) were independent risk factors for death in ARDS children. Conclusions:miR-424 can help with early diagnosis of sepsis complicated with ARDS, and can be used as a predictor of the prognosis of sepsis children complicated with ARDS.

With the development of society and economy, the disease spectrum of Chinese residents has undergone a major change, and the public health emergencies have occasionally occurred. The new situation has put forward higher requirements on the emergency capacity, professional technical level and treatment quality of critical care treatment system. Combining with the advantages of hospital aviation, ground and Internet emergency, Henan Provincial People's Hospital established a new construction path of emergency and critical care treatment system, namely "trinity" rescue system of aviation, ground and Internet emergency treatment and the "five-ring" model of critical care treatment system in hospital. The purpose of this paper is to summarize the practical experience of the construction of emergency and critical care system in Henan Provincial People's Hospital, so as to provide reference for the management of emergency and critical care in China.

OBJECTIVE: To investigate the improvement effects of Qishenlian eczema cream on the atopic dermatitis (AD) model guinea pigs, and to investigate its possible mechanism. METHODS: Female guinea pigs were randomly divided into normal control group, model control group, matrix control group (Qishenlian eczema cream matrix, 1 g/kg), Qishenlian eczema cream low-dose, medium-dose and high-dose groups (0.5, 1, 2 g/kg) and Hydrocortisone butyrate cream group (0.5 g/kg), with 10 guinea pigs in each group. Using ovalbumin as antigen, the skin allergy of guinea pigs was stimulated to induce AD model. Forty-eight hours after the last excitation, guinea pigs were smeared with normal saline in normal control group and model control group, while those were smeared with Qishenlian eczema cream matrix in matrix control group, and administration groups were treated with relevant medicine, twice a day, for consecutive 14 d. The scores of erythema, edema and scratch before and after administration were recorded in each group. The morphological characteristics of skin were observed by HE staining. TUNEL method was used to detect the apoptosis of keratinocytes in skin tissue. The level of IFN-γ in skin lesions was detected by ELISA. RESULTS: Compared with normal control group, inflammatory cell infiltration, hyperkeratosis of epidermis and thickening of spinous layer were observed in skin tissue of guinea pigs in model control group and matrix control group; the scores of erythema, edema and scratch and their total score, the level of IFN-γ in skin tissue were increased significantly; the apoptosis number of keratinocytes was decreased significantly (P<0.01). Compared with model control group and matrix control group, above symptoms of medication groups were relieved to different extents; the scores of erythema, edema and scratch and their total score after medication in medication groups, the level of IFN-γ in skin tissue in Qishenlian eczema cream medium-dose and high-dose groups and Hydrocortisone butyrate cream group were decreased significantly, and above scores in medication groups were significantly lower than before medication (P<0.05 or P<0.01); the apoptosis number of keratinocytes was increased significantly in medication groups (P<0.05 or P<0.01). CONCLUSIONS: Qishenlian eczema cream has certain improvement effect on AD model guinea pigs, the mechanism of which may be associated with promoting the keratinocyte apoptosis, reducing the level of INF-γ in skin tissue.

Osteoporosis is a systemic metabolic degenerative skeletal disease, characterized by reduced bone mass and disrupted bone microarchitecture, leading to enhanced bone fragility, decreased bone strength and increased susceptibility to frac?tures. Its occurrence and development is an extremely complex biological process, which is a multi factor, multi gene and multi stage experience. Osteoporosis is the fourth chronic disease in our country, which mainly affects postmenopausal women and the el?derly. Bone mass is jointly maintained by the action of bone resorption by osteoclasts and bone formation by osteoblasts. If the im?balance between the two occurs, resulting in reduced bone formation and increased bone resorption, bone homeostasis is changed to cause osteoporosis. Traditional treatment for osteoporosis are drugs which just to depress bone resorption (such as bisphospho?nates and estrogen), or just to promote bone formation (such as PTH and statins), or just to supply the source of calcium. This sin?gle point of local treatment is clearly difficult to play a long?term part in complex lesions of osteoporosis. In recent years, research?ers have studied the molecular signaling pathways related to osteoporosis. The current studies reveal that signaling pathways in os?teoporosis include MAPK, PI3K/Akt, Wnt/β?catenin, RANKL/RANK/OPG, Hedgehog and Notch signaling pathways. These signal?ing pathways play an important role in the molecular pathogenesis of osteoporosis. In addition, there are GH/IGF?1, PPAR?γ, PTH signaling pathways and so on. Taking these signaling pathways as targeting points is a new strategy for the development of new anti osteoporosis drugs or the prevention of osteoporosis. But there are cross?talks and/or interactions between these signaling pathways related to osteoporosis and the pathogenesis of osteoporosiswhich make its therapy more difficult. This paper reviews the research progress of signaling pathway in osteoporosis in order to explore the molecular mechanism of its occurrence. Thus these signaling pathways can be taken as new targets to develop safer and more effective anti?osteoporosis drugs.

Objective To explore the clinical effect of ultrasound-guided ilioinguinal/iliohypo-gastric nerve blocks marked by arteriae circumflexa ilium profunda in elderly hernia surgery. Methods Forty ASA Ⅰ-Ⅲ grade patients (33 males and 7 females)of 65-90 years old scheduled for elective hernia surgery were randomly divided into two groups (n =20).In group T,patients received ilioinguinal/iliohypogastric nerve blocks bytraditional anatomical positioning;in group V,patients re-ceived ultrasound-guided ilioinguinal/iliohypogastric nerve blocks marked by arteriae circumflexa ilium profunda.The comparison was made between the two groups in term of onset time of anesthe-sia,VAS score of intraoperative and postoperative 6 h.Anesthesia satisfaction,incidence of uros-chesis,misplacement local anesthetics into blood-vessels were recorded.Results The onset time of anesthesia in group V was significantly shorter than that in group T [(6.1 ± 1.8)min vs (12.1 ± 2.0)min,P <0.05].The VAS score of intraoperative in group T was significantly higher than that of group V [(4.5 ± 1.1 )scores vs (2.1 ± 0.9 )scores,P < 0.05 ].The anesthesia satisfaction of group V was higher than that of group T (P <0.05).There was one misplacement local anesthetics into blood-vessels in group T.Conclusion Ultrasound-guided ilioinguinal/iliohypogastric nerve blocks marked by arteriae circumflexa ilium profunda can provide safe,effective and reliable anesthesia in elderly hernia surgery.

Objective To explore the apphcation of the integrated teaching method of PBL and TBL based on cases in pharmacology teaching.Method Ninety-two students majored in Pharmaceutical Preparations were selected into the control group,and 184 students majored in pharmacy were enrolled into the experimental group.The teaching effect was comprehensively evaluated by using the combination of theoretical examination and student feedback.Results In the experimental group,the average score of students in Pharmacology (73.68 ± 9.40) was significantly higher than that of the control group (67.34 ± 7.18),and the feedback of the students in the experimental group was significantly better than that in the control group.Conclusion Compared with the traditional teaching method,the integrated teaching method can significantly improve the students' theoretical examination results and students' learning interest.

Objective To investigate the effect of geraniin on expression of Wnt3a protein and mRNA in bone marrow stromal cell (BMSC) from osteoporotic rats. Methods The model of osteoporosis (OP) was duplicated by ovariectomy in rats. BMSCs were isolated and cultured. BMSCs from shamed rats were routinly cultured and taken as normal control, and BMSCs from OP rats were divided into model group, 1μmol/L simvastatin positive group, and geraniin group (0.01, 0.1, 1, 10, 100 μmol/L), respectively. The methods of realtime-PCR and western blot were used to assay the protein and mRNA expression of wnt3a, respectively.Results As compared with normal control group, the protein and mRNA expression of wnt3a in model group were significantly suppressed;Compared with model group, 1 μmol/L simvastatin, and 0.1, 1, 10 and 100 μmol/L geraniin significantly increased the expression of wnt3a protein and mRNA. Conclusion It is suggested that geraniin activates wnt/β-catenin pathway though increasing the expression of signaling protein wnt 3a in BMSCs from OP rats. It would be beneficial to osteogenic differentiation of BMSCs and osteogenesis.

Secretory diarrhea provides a major health challenge worldwide, which is one of the most important reasons for children morbidity and death. The activation of Cl- channels in intestinal epithelial cells resulting in the excessive fluid secretion in the intestine is the main reason of diarrhea caused by enterotoxins. In diarrhea caused by cholera and the other bacterial enterotoxins, cystic fibrosis transmembrane conductance regulator ( CFTR) is the main cAMP-control Cl- channel to promote the fluid secretion in epithelial cells. Therefore, CFTR inhibitors are the new choices for secretory diarrhea. CFTR inhibitors include thiazolidinone, glycine hydrazide and quinoxalinedione chemical classes, and some components from natural plants also exhibit CFTR inhibition activity, however, further studies should be done.

Objective To investigate the differences of protein and mRNA expression of matrix metalloproteinase 9 (MMP-9) in osteoclasts (OC) and osteoclast-like cells (OLC) obtained by mechanical and inducement methods. Methods Mechanical separation method was used to separate mature osteoclasts from long bones of SD rats aged one-day;and inducement culture method was applied to induce OLC by using RANKL (100 ng/mL) and M-CSF (100 ng/mL) . The protein expression of MMP-9 was measured by immunocytochemistry and mRNA expression of MMP-9 was assayed by in situ hybridization. Results The integral optical density (IOD) and average optical density (AOD) of positive cells in the visual field were higher in the 12-d group of inducement method as compared with the 3 d-group of mechanical method. Conclusions It is suggested that the protein and mRNA expression of MMP-9 in OLC obtained by 12 d inducement method is much high than in OC obtained by 3 d mechanical method. OLC obtained by inducement method can be applied in the study of osteoporosis.

Objective To investigate the 64-slice spiral CT angiography combined with negative contrast CT cholangiography in the evaluation of resectability of hilar cholangiocarcinoma.Methods The clinical data of 46 patients with hilar cholangiocarcinoma were analyzed.The resectability was observed by the 64-slice spiral CT angiography combined with negative contrast CT cholangiography,and compared with the postoperative results.Results Preoperative CT Bismuth-Corlette classification and postoperative results showed no significant difference (x2 =0.00,0.53,1.96,0.59,2.63,all P > 0.05).Preoperative CT assessed the resectability of 26 cases with hilar cholangiocarcinoma,The intraoperation evaluated resectable 22 cases,the difference was not significant (x2 =1.51,P > 0.05).Conclusion 64-slice spiral CT angiography combined with negative contrast CT cholangiography can make a good evaluation of hilar cholangiocarcinoma involving the degree of blood vessels and bile ducts,provide an important basis for the selection of clinical protocols.

Objective To investigate the effects of geraniin on platelet aggregation and platelet-neutrophil interactions.Methods Platelet aggregation,in vitro and ex vivo,was determined by use of Born's method,and the binding of thrombin-stimulated platelets to neutrophils was observed based on the rosette assay.Intracellular calcium concentration of platelets was measured by using Fura-2-AM.Results Geraniin in vitro significantly inhibited arachidonic acid (AA)-,adenosine diphosphate (ADP)-,or platelet activating factor (PAF)-induced platelet aggregation,in a concentration-dependent manner.The medium inhibitory concentrations (IC50) were 2.4,0.4 and 1.1 μmol/L,respectively.Intragastric geraniin at 5 mg/kg markedly suppressed platelet aggregation induced by AA,ADP,or PAF.Geraniin decreased the total rise of [Ca2+]i,Ca2+ release,and Ca2+ influx,in a concentration-dependant manner.The IC50 values were 71.9,84.9,and 62.9 μmol/L,respectively.Geraniin decreased the binding of thrombin-stimulated platelets to neutrophils,and significantly inhibited washed platelet aggregation stimulated by fMLP-activated neutrophils.The IC50 values were 3.2 and 10.2 μmol/L,respectively.Conclusion It is suggested that geraniin inhibited platelet aggregation in vitro and ex vivo,decreased the calcium mobilization of platelets,and suppressed the interactions between platelets and neutrophils.

Objective To construct reference standards for detection and quantification of Klebsiella pneumoniae (K.pneumoniae) with SYBR Green I-based real-time PCR assay. Methods Primers were designed based on the published sequence of the phoE gene of K.pneumoniae. The standard was prepared by cell culture, PCR and T-A clone methods, and was identified by colony PCR and DNA sequencing. Results The standard curve showed a very good linear negative regression between threshold cycle (Ct) and Log starting quantity of copy number. The detection range was from 5.2 to 5.2×106 copies per reaction, and the detection limit was 6 copies per reaction. The coefficients of variance (CVs) of three parallel experiments were in the range of 0.05%-0.91%. Conclusion The reference standards have high stability and reproducibility. They can be used in the quantitative detection of K.pneumoniae.

<p><b>OBJECTIVE</b>This study investigated the thrombolitic effect of Tianma Xinnao capsule and its preliminary mechanism, as well as the effect on coagulation system.</p><p><b>METHOD</b>Charlton's and Tomihisa's methods were modified to investigate the thrombolytic effect of Tianma Xinnao capsule. The activities of type 1 plasminogen activator inhibitor (PAI-1) and tissue-type plasminogen activator (tPA) in rabbit plasma were assayed by use of ELISA. The effects of Tianma Xinnao capsule were also evaluated on euglobulin lysis time (ELT), prothrombin time (PT), kaolin partial thromboplastin time (KPTT), thrombin time (TT), fibrinogen (Fib) and hemorheology.</p><p><b>RESULT</b>The results showed that Tianma Xinnao capsule had a dose-dependent thrombolytic effect in rats. Tianma Xinnao capsule at 0.6 and 1.2 g x kg(-1) produced 40% and 50% of reperfusion rate, while obtained 50% and 40% of reocclusion rate; 0.3 g x kg(-1) of Tianma Xinnao capsule, however, had no effect on the reperfusion or reocclusion rate. Tianma Xinnao capsule significantly inhibited PAI-1 activity, while elevated tPA activity in rabbit plasma. Tianma Xinnao capsule markedly prolonged ELT, PT, and KPTT, and decreased Fib level. Tianma Xinnao capsule showed no significant influence on TT or high, medium, or low sheering viscosity.</p><p><b>CONCLUSION</b>It is indicated that Tianma Xinnao capsule inhibited PAI-1 activity and increased tPA activity, and this property of Tianma Xinnao capsule is assigned to be responsible for the thrombolytic effect.</p>

Objective To study the protective effects of Tianma xingnao capsule on brain blood circulation in mice and rats.Methods The arteria carotis externa was ligated,and arteria carotis interna was kept.The left carotid artery was exposed and placed over an ultrasonic flow probe.The brain flow volume was recorded with an Ultrasonic Volume.The pursiness time of mice was observed to evaluate the effect of Tianma xingnao capsule on cerebral ischemia and anoxia.The permeability of normal blood-cerebral barrier was investigated in mice.Results Tianma xingnao capsule significantly increased brain blood flow and prolonged mouse pursiness time.Tianma xingnao capsule at a dose of 4 g/kg significantly elevated the content of Evans in mouse brain.Conclusions Tianma xingnao capsule can increase the brain blood flow in rats and improve brain blood circulation in mice.It is obviously advantageous to protect against cerebral ischemia and hypoxia.

Objective To investigate antithrombotic and antiplatelet effects of Dragon's Blood,?-cyclodextrin inclusion of Dragon's Blood.Methods The methods of injection of adenosin diphosphate(ADP) into mouse tail vein,electrically stimulated arterial thrombosis in rats were used to evaluate the anti-thrombotic effects of Dragon's Blood and?-cyclodextrin inclusion of Dragon's Blood,respectively.Platelet aggregation(PRP or washed platelets) was tested according to Born's method.Results ?CYDB significantly protected against thrombosis caused by ADP on mice,and electrical stimulation on rats.In vitro?-cyclodextrin inclusion of Dragon's Blood inhibited AA,ADP,PAF-induced platelet aggregation.Conclusions The results suggest that compared with Dragon's Blood,?-cyclodextrin inclusion of Dragon's Blood has more potent anti-thrombotic effects on the thrombotic models,its mechanisms may be closely related to the inhibition of anti-platelet aggregation.?-cyclodextrin inclusion of Dragon's Blood can improve the bioavailability of Dragon's Blood.

OBJECTIVE: To investigate the effects of PUW (a fraction containing 60% corilagin from a Chinese herbal plant Phyllanthus urinaria) on thrombosis and coagulation system. METHODS: Myers's method was used to evaluate the protection of intravenously administered PUW against the male mouse sudden death caused by injection of 75 mg/kg arachidonic acid in the tail vein; Charlton's method was modified to observe antithrombotic effect of PUW in rat electrically stimulated carotid artery thrombosis model; and the model of venous thrombosis was produced to investigate the antithrombotic effect of PUW. Rosette assay was used to observe the effect of PUW on platelet-neutrophil adhesion. The effects of PUW were monitored on euglobulin lysis time (ELT), prothrombin time (PT), kaolin partial thromboplastin time (KPTT), and bleeding time (BT) in rats, according to the methods of Kowalski, HUANG Zheng-Liang, and GU Yue-Fang, et al, respectively. RESULTS: The results showed that PUW administered intravenously significantly decreased the mouse mortality, prolonged the occlusion time of rat carotid arteries, and reduced the wet and dry thrombus weight of the inferior vena cava, respectively. PUW markedly inhibited the binding of activated platelets to neutrophils, obtaining 39.7 mg/L of the medium inhibitory concentration. Intravenously administered PUW significantly shortened ELT, prolonged KPTT while had no influence on PT; PUW increased BT in rat tail tips but the BT caused by PUW was much shorter than that by aspirin or urokinase. CONCLUSION: It is suggested that PUW has the potential of antithrombosis due to its inhibition of platelet-neutrophil adhesion. PUW shows the tendency to bleeding, however, it could not cause serious bleeding side effect as compared with aspirin or urokinase.

Aim To explore the effects and its mechanisms of ginsenoside-Rg1 on level of t-PA and PAI-1.Methods Type 1 plasminogen activator inhibitor(PAI-1) and tissue type plasminogen activator(t-PA) activity in plasma were assayed using chromogenic substrate.Results The results showed that ginsenoside-Rg1 in vitro or in vivo significantly inhibited PAI-1activity,while increased t-PA activity.These effects were concentration-dependent.Intravenous Panax notoginsenoside Rg1 at 30,60,120 and 240 mg?kg~(-1) markedly suppressed PAI-1 level in plasma as well as platelet-released substances stimulated by thrombin,while increased plasma t-PA activity.And release level of PAI-1 owing to blood platelet was greatly decreased by ginsenoside-Rg1.Conclusion Ginsenoside-Rg1 showed potent antithrombosis due to the inhibition of PAI-1 and increase of t-PA.It might also be a advantagous mechanism to its antithrombsis.

Objective To analyze the condition, distribution, constituent ratio, main symptoms, laboratory examination and other clinical features of juvenile rheumatic disease (JRD) in order to enhance theunderstanding of it. Methods Five hundred and sixty-one cases of JRD treated in the Department of Pediatrics of Henan Provincial People′s Hospital were analyzed retrospectively from January 1990 to August 2003. Results Henoch-Schonlein purpura (HSP), juvenile idiopathic arthritis(JIA), rheumatic fever (RF), systemic lupus erythematosus (SLE) occurred more in rural area than in urban. Mucocutaneous lymphnode syndrome (MCLS) occurred more in urban than in rural area.The initial symptoms of JRD were arthralgia, skin rashes and fever. The frequently involved organs were kidney, lymphnode, heart and liver. White blood cell count and erythrocyte sedimentation rate raised in 43.1% and 61.6% patients with JRD respectively. Rheumatoid factor was mostly positive in JIA. Three patients died. Conclusion 2.9% patients with JRD have been admitted to Department of Pediatrics. The most common diseases are HSP, JIA, MCLS, RF, SLE. The morbidity of HSP shows an increasing tendency and RF shows decreasing tendency. It is suggested to enhance the understanding of JRD. Improve the diagnostic and treatment level is the key to control disease and reduce complications.
